Potential Strategies

  • Establish shared data definitions, collection standards, and processing rules across systems to enable EMR and EHR interoperability, consistent reporting, and reliable statewide analysis.

  • Implement a closed-loop referral system that allows referrals to be sent, received, accepted, and tracked across providers, supporting warm handoffs, follow-up, and accountability during transitions in care, all while maintaining consumers’ confidentiality.

  • Develop dashboards for system leaders and policymakers to track access, capacity, utilization, and outcomes across regions and populations to inform planning and investment decisions.

  • Equip providers, navigators, and consumers with timely, actionable information on service availability, eligibility, and care options to support coordination and reduce delays in connecting to care.
